Family Practitioner Role

Centacare's Family Mental Health Support Service offers early intervention support to vulnerable families with children and young people aged 0-18 years who are showing early signs of, or are at risk of developing, mental illness.
The program provides outreach services to rural and remote regions in South Australia, including Murray, Mid Murray, Coorong and Mallee. Our service is client-centered, using strength-based approaches within a trauma-informed care model.
As a Family Practitioner, you will provide one-on-one counselling for clients and their families in their homes, schools or offices, and work in partnership with them to develop a Family Action Plan.
Key Responsibilities:
- Provide one-on-one counselling for clients and their families
- Work with clients and their families to develop a Family Action Plan
- Collaborate with other professionals to provide comprehensive support
Requirements:
- Degree in Social Work or a related discipline
- Experience working with vulnerable families with children and young people
- Valid Department of Human Services (DHS) Working with Children Check
- Valid driver's licence
