
Clinical Lead – Patient Care

This role involves working with patients who have long-term conditions and complex needs, providing clinical case management to help them remain at home as appropriate.
- Main Responsibilities:
- Maximise independence by supporting individuals with complex health needs in their own homes.
- Conduct comprehensive assessments and develop individualised care plans in collaboration with healthcare teams.
- Link with existing services to facilitate early discharge from hospital and prevent re-admission.
The successful candidate will possess a strong understanding of clinical assessment, diagnosis, and treatment. They will be able to conduct comprehensive health and social care assessments, including medical history, physical examinations, medication review, prescribing, referrals, and functional/cognitive assessments.
- Health Assessments: Conduct thorough health assessments for patients within the defined group.
- Medication Management: Review and manage patient medication, including prescriptions and referrals.
- Care Planning: Develop, monitor, and manage care plans in collaboration with primary healthcare teams.
The ideal candidate will possess excellent leadership and communication skills, enabling them to effectively manage a team and collaborate with other healthcare professionals. Key responsibilities include:
- Managing complex clinical interventions on an ongoing basis.
- Undertaking risk assessments and monitoring changing situations without recourse to others where possible.
- Ensuring safe service delivery and line managing a defined team of staff.
