We are seeking a highly skilled and compassionate Diabetes Educator and Care Coordinator to join our team at St Vincent's Hospital Melbourne. As a key member of our Health Independence Program, you will play a vital role in delivering high-quality care to clients with complex medical and social needs.
Key Responsibilities:
- Provide diabetes specialist assessment and implement clinical management strategies to improve health outcomes and prevent avoidable hospital presentations.
- Conduct comprehensive holistic assessments, coordinate all aspects of health and social care needs, and monitor achievements of client goals.
- Facilitate intervention from other healthcare professionals and ensure appropriate linkages with hospital and community services.
- Work collaboratively with clients, carers, and families to develop care plans and achieve health and well-being goals.
Requirements:
- Current nursing registration with AHPRA.
- Postgraduate Certificate in Diabetes Education (credentialed or working towards).
- Current driver's license.
- Current National criminal history check, or willingness to obtain.
