About the Opportunity

We are seeking a highly skilled and compassionate Senior Youth Care Coordinator to join our team at Melbourne City Mission. As a key member of our Health Care for the Homeless (HfH) Program, you will play a critical role in supporting young people aged 12 to 24 who are experiencing or at risk of homelessness.

Key Responsibilities
  • Provide clinical leadership for the SVHM Check-In staff based at Frontyard Youth Service, ensuring delivery of an efficient, effective, and clinically therapeutic service.
  • Manage a small caseload of clients within the Check-In program and provide brief opportunistic support to users of the drop-in service.
  • Work collaboratively with Frontyard, SVHM, and team leadership to ensure delivery of an effective and responsive service for clients of the Check-In program.
  • Screen and engage with clients within the accommodation service and drop-in centre, using holistic comprehensive assessments based on the clients' health and wellbeing needs.
About You

We are looking for someone who:

  • Has a nursing qualification with a minimum of 7 years' experience in a relevant acute or community setting.
  • Is eligible for registration/membership with AHPRA.
  • Has previous experience in a similar or related role, including working within a care coordination framework with people with complex psychosocial needs.
  • Has knowledge and experience in one or more of the areas of homelessness, mental health, substance use, young people with complex needs, and/or disability.
  • Has demonstrated understanding of issues impacting youth health and wellbeing.
  • Has highly developed interpersonal skills, including excellent verbal and written organisational and negotiation skills.
