We are seeking a highly skilled and compassionate Diabetes Educator/Care Coordinator to join our team at St Vincent's Hospital Melbourne. As a key member of our Complex Care and Chronic Disease team, you will play a vital role in delivering high-quality care to patients with complex medical and social needs.
Key Responsibilities:
- Provide diabetes specialist assessment and implement clinical management strategies to improve health outcomes and prevent avoidable hospital presentations.
- Conduct comprehensive holistic assessments, coordinate all aspects of health and social care needs, and monitor achievements of client goals.
- Facilitate intervention from other healthcare professionals and ensure appropriate linkages with hospital and community services.
- Work collaboratively with clients, carers, and families to develop care plans and goals using motivational interviewing and self-management approaches.
- Identify and access multidisciplinary and specialist supports to address specific client issues.
- Monitor health outcomes and plan for transition and exit from the program.
Requirements:
- Current nursing registration with AHPRA.
- Postgraduate Certificate in Diabetes Education (credentialed or working towards).
- Current driver's license.
- Current National criminal history check, or willingness to obtain.
