Overview

Clinical Nurse Specialist - Palliative and Supportive Care at Nepean Blue Mountains Local Health District (NBMLHD). The CNS2 role delivers compassionate, coordinated care within the Supportive and Palliative Care service across NBMLHD, guiding patients and carers through the care continuum with dignity and support from hospital to home.

Responsibilities
  • Deliver advanced clinical nursing care with autonomy and in collaboration with multidisciplinary teams.
  • Provide care coordination to ensure a seamless journey through the care continuum from hospital to home.
  • Champion, educate and lead a multidisciplinary team to improve patient outcomes.
  • Demonstrate problem solving and decision-making according to best practice.
  • Communicate effectively with patients, family/carers and a diverse range of health care providers.
Qualifications
  • Current registration with AHPRA – Division 1 Registered Nurse.
  • Post-registration qualifications in palliative care.
  • At least 3 years full-time equivalent experience in palliative care nursing.
  • Current Driver's License and willingness to use for work-related travel (may include long distances across the LHD).
