About Our Support Coordinator Role

We are seeking a highly skilled and motivated Support Coordinator to join our professional team. In this role, you will be responsible for delivering high-quality coordination of supports to participants in line with their goals and NDIS expectations.

  • Deliver Coordination of Supports (Level 2) and Specialist Support Coordination while prioritizing quality service, choice, and control, and participant empowerment.
  • Manage a caseload independently while working collaboratively with the team to achieve shared objectives.
Key Responsibilities:
  • Assist participants to understand and implement their NDIS plans, engage providers, and resolve service or access issues.
  • Build and maintain strong, respectful relationships with participants, families, support networks, and providers.
Requirements:
  • Experience in Support Coordination under the NDIS (Level 2 or Specialist).
  • Relevant qualifications in human services, social work, community services, or allied health.
  • Strong communication, organisation, and problem-solving skills.
