Band 6 Occupational Therapist – Care Coordinator

What you will do:

- Manage a caseload, providing evidence-based, person-centred assessments and interventions.
- Work collaboratively within a multi-disciplinary team to enhance service delivery.
- Contribute to service improvement initiatives focused on recovery and independence.
- Support service users in achieving their recovery goals, promoting independence, and addressing physical health needs.
- Liaise closely with local services and primary care providers to ensure holistic care.

Qualifications and experience required:

- Registered Occupational Therapist with a degree-level education.
- Prior experience in mental health settings and multi-disciplinary team environments.
- Demonstrated capability to work with complex and challenging service users.
- Strong leadership skills, with the ability to supervise junior staff.
- Comprehensive knowledge and experience of integrated assessments, care planning, and risk assessment.
- Desirable qualifications include being a car owner and holding a current driving licence.
