We are seeking a dedicated and experienced Clinical Care Coordinator to join our team at Bayview Haven in Portland, Victoria, on a Full-time basis. Situated along Victoria’s beautiful southwest coast, Portland offers a relaxed seaside lifestyle, a welcoming community spirit, and easy access to stunning beaches, local attractions, and everyday conveniences. It’s the perfect balance of coastal tranquility and modern living.
About the Role
As Clinical Care Coordinator, you will play a vital role in ensuring our residents receive the highest standard of person-centred clinical care. You will:
- Assess resident needs and develop tailored care plans.
- Lead and support a dedicated care team to deliver safe, high-quality care.
- Maintain accurate clinical documentation and oversee compliance requirements.
- Provide hands-on nursing care, including wound management, medication administration, and monitoring.
- Communicate effectively with residents, families, and health professionals.
- Support infection control measures, clinical audits, and quality improvement initiatives.
- Act as a role model and mentor for nursing and care staff.
About You
We are looking for someone who brings:
- Bachelor of Nursing and current AHPRA registration.
- Experience in residential aged care nursing.
- Strong clinical assessment, documentation, and leadership skills.
- Excellent communication and interpersonal skills.
- Computer literacy, including the use of Microsoft Office and clinical software.
