Where you'll be working

Ryde Community Mental Health Centre – 39‑41 Fourth Avenue Eastwood; RNS Community Centre – 2C Herbert Street, St Leonards. Ryde Hospital is having an incredible $479 million complete hospital redevelopment that will transform the delivery of healthcare for the Ryde local community. The redevelopment will result in new health facilities and enhanced services including emergency, ICU/critical care, inpatient units, outpatient/ambulatory care services and much more. Construction commenced in early 2023, providing you with a wonderful and exciting opportunity to be part of the Ryde team to change the future health services for the Ryde Community completely.

What you'll be doing

Provision of care coordination for older people who present with complex mental health issues across 2 sites. This may include physical comorbidities, psycho‑social challenges and issues of addiction and substance use. The successful applicant will be required to provide education and guidance to internal and external partners including residential aged care facility staff.

People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.

Candidate requirements

- Registered Nurse who has a minimum of 5 years full‑time postgraduate experience in a mental health specialty. Additionally, the applicant must have an approved postgraduate qualification within the speciality of mental health drug and alcohol or other such qualifications or experience deemed appropriate.

- Demonstrated experience working with consumers and their carer/s presenting with complex mental health needs and/or behaviours of concern and demonstrated ability to assess, plan, implement and evaluate interventions and care plans.

- Extensive experience in conducting comprehensive mental health assessments and developing, implementing and evaluating consumer focused comprehensive care plans based on evidence‑based practice.
