Hearts at Home is a new initiative supporting individuals living with heart failure, enabling early discharge from hospital and reducing avoidable admissions. You’ll provide high-quality, evidence-based clinical assessment, care planning, intervention, education, and management—leveraging virtual care and remote monitoring technology to make a real difference in people’s lives.You’ll work closely with hospital and primary care teams, using your expertise to coach clients in self-management, coordinate care, and ensure seamless transitions.
