We are looking for a full-time Clinical Pharmacist (part-timealso considered) to join our highly valued and well-integrated team across GrandUnion PCN. Our pharmacists play a vital role in medicines optimisation, workingclosely with GPs, Pharmacy Technicians, and other healthcare professionals toimprove patient outcomes and reduce risks associated with medicines.

Whether you're already experienced in primary care orlooking for a new challenge, you'll receive full induction, mentorship, andprotected time to develop your skills, including funded access to the CPPEPrimary Care Pharmacy Education Pathway (PCPEP) and the Independent Prescribingqualification.

Main duties of the job

As our Clinical Pharmacist, you'll be a key member of the practice team, working across practices in Grand Union PCN. This varied and rewarding role includes delivering structured medication reviews, supporting care home residents, and responding to medicines-related queries from patients, GPs, and colleagues with clear advice and follow-up.

You'll collaborate closely with Pharmacy Technicians and build strong links with local prescribing teams, community pharmacies, hospitals, the Primary Care Training Hub, and the ICS to provide high-quality medicines support and contribute to initiatives such as the Prescribing Achievement Framework.

We're also committed to developing future pharmacy professionals. The team supports trainee pharmacists and pharmacy technician apprentices in partnership with NGH, with everyone playing a role in mentoring and training students, new staff, and other healthcare professionals.

Balancing hands-on patient care, service development, education, and medicines optimisation, this is a role where your expertise directly improves patient outcomes while shaping the next generation of pharmacy talent.

Job responsibilities

Conduct Structured MedicationReviews (SMRs) for patients with single or multiple medical conditions,identifying and prioritising patients according to clinical risk and contractrequirements.

Review ongoing medicine needs,monitoring requirements, and opportunities for medicines optimisation, makingrecommendations to clinicians or implementing changes as appropriate.

Involve patients in decisions about their prescribedmedicines and support adherence in line with NICE guidelines.

Work with care home staff toimprove the safety of medicines ordering and administration.

Provide patient-facing andtelephone clinics for those with questions, queries, and concerns about theirmedicines in the practice.

Respond to medicine-relatedenquiries from GPs, practice staff, community pharmacies, and other healthcareteams, providing advice, solutions, and follow-up to patients to monitor theeffect of any changes.

Review medicines associated withunplanned hospital admissions and readmissions through audits and individualpatient reviews, implementing changes to reduce the prescribing of thesemedicines to high-risk patient groups.

Identify cohorts of patients athigh risk of harm from medicines through pre-prepared practice computersearches. This might include risks that are patient-related, medicine-related,or both.

Contribute pharmaceutical expertiseto the development and implementation of new services that have medicinalcomponents, including treatment pathways and patient information leaflets.

Analyse, interpret and presentmedicines data to highlight issues and risks to support decision making.

Undertake simple audits ofprescribing in areas directed by the GPs, provide feedback on the results, andimplement changes in conjunction with the practice team.

Implement changes to medicinesthat result from MHRA alerts, product withdrawal and other local and nationalguidance.

Provide education and training to theprimary healthcare team on therapeutics and medicines optimisation.

Work with the general practiceteam to ensure the practice is compliant with CQC standards where medicines areinvolved.

To support public healthcampaigns and provide specialist knowledge on all public health programmesavailable to the public.

Person Specification

Qualifications

- Registration with General Pharmaceutical Council
- Completion of CPPE PCPEP (or equivalent) pathway, or willingness to undertake this.

Job Circumstances

- Reliable transportation and ability to travel across the locality on a regular basis, including carrying out home visits.
- Possess the legal right to work in the UK and ability to provide appropriate documentation for verification.

Experience

- Understanding of general practice and primary care prescribing, including rational prescribing strategies for improvement.
- Experience in planning, managing, monitoring, and reviewing medicine optimisation for long-term conditions.
- Ability to work under pressure, meet deadlines, and produce timely, informative reports.
- Proven track record of working independently and collaboratively within a team.

Skills and Knowledge

- Excellent interpersonal, influencing, negotiation, written, and verbal communication skills, including explaining complex or sensitive information to varied audiences.
- Strong IT skills and ability to obtain and analyse complex technical information.
- Ability to prioritise, problem-solve, recognise deviations, and escalate appropriately.
- Ability to gain acceptance for recommendations and influence others to follow agreed actions.
- Accountability for delivering professional expertise and direct service provision.
