The Team: The Mental Health Inpatient Unit (MHIU) is located in the Royal Hobart Hospital, level 2 and 3K West. MHIU sits under the Mental Health Department (MHD) within the Statewide Mental Health Services' Acute Care Stream and supports adults with serious mental illness across multiple locations, serving southern Tasmania and those best suited to its services. The Clinical Nurse Specialist – Discharge Planner plays a vital role in supporting safe, timely, and coordinated discharges within a high-demand acute mental health setting. As an integral member of the Multidisciplinary Team, the CNS works collaboratively with treating teams, clinicians, and consumers to ensure person-centred, recovery-oriented discharge planning. The role involves building strong relationships across the broader service network, including NDIS providers and accommodation services, to facilitate smooth transitions to the least restrictive and most appropriate care environment. You will need: - Demonstrated ability to deliver high quality mental health care in an acute inpatient mental health setting with a particular focus on discharge planning assessments, risk assessments, brief psychological interventions and developing management plans. - Extensive demonstrated experience working in an inpatient mental health setting in an autonomous role and as part of a multidisciplinary team. - Demonstrated knowledge of the psychological and healthcare needs of individuals, including the understanding of recovery principles, policies and procedures, and associated legislation which impact upon inpatient and community based settings. Salary:$106,570 to $112,440 per annum(Pro rata). Our Employer 12% superannuation contribution is on top of this amount. - Salary range is in accordancewith Nurses and Midwives (Tasmania State Service) Agreement 2023. In addition, the following allowances calculated on the salaried incremental point may be available: - Post Graduate Allowance of up to 10% based on qualification type Salary Packaging: You’ll have access to salary package a range of benefits including living expenses up to $9,010 + $2,650 in meal entertainment each FBT year + other benefits if eligible.
