Overview

You will be responsible for the provision of skilled nursingcare to the population of York Central locality. You will deliver care within the boundaries ofyour role, focusing on supporting patients to be healthy, monitoring long termconditions, health prevention and screening activities. You will workcollaboratively to meet the needs of patients, ensuring the highest standardsof care, supporting the delivery of policy and procedures. You must always work within your own competence andprofessional Code of the Nursing and Midwifery Council (NMC).

Main duties of the job

Clinical responsibilities:

- Assess plan, develop, implement and evaluate programmes of care to improve health and well-being and meet the healthcare needs of individuals.
- Perform and interpret spirometry (ARTP Registered)
- Perform routine nursing tasks: Clinical observations, Height/ Weight
- Provide a holistic approach and deliver evidence-based care.
- Administer and monitor medication consistent with protocols, standards and legislation.
- Enable patients, families and carers to develop their knowledge and understanding about their health and well-being and be as self-managing as possible.
- Prioritise health problems and intervene appropriately to assist patients in complex, urgent or emergency situations, including initiation of effective emergency care.
- Assist in the provision of nursing care within the context of primary care for patients with mental health problems.
- Offer advice about adult and child vaccinations.
- Delegate aspects of care clearly and appropriately, adopting the principles of safe practice and assessment of competence.
- Ensure that all daily checks are recorded accurately and in a timely manner e.g. Spiro/FeNo equipment, defibrillator, emergency equipment, oxygen.

Clinical responsibilities:

Assistwith the maintenance of equipment and report defective equipment

Takeresponsibility that any equipment either clinical or otherwise, used whenworking, is taken due care of and locked away at the end of every workingsession.

Maintain stock levels ofclinical equipment and supplies, reporting the Reception Team as required.

Assist insetting up the Community Diagnostic Centre at the start of the session andensure equipment is appropriately stored at the end of the shift.

Act as a positive role model.

Maintain ethos and culture of Nimbuscare.

Positively promote Nimbuscare.

Support Line Managers as required.

Adhere toHealth & Safety and educate staff within your role.

Apply Nimbuscare policies,standards and guidance.

Attend in-house governance,educational and staff meetings as appropriate.

Suggest and support theintroduction of new working processes to optimise quality.

Communicateto ensure that all employees operate as an effective team.

Build and maintainrelationships with other healthcare professionals, including GPs, GP surgerypersonnel, care home personnel, CCG personnel, Students.
