As a key member of a multi-disciplinary outreach healthcare team, you will deliver trauma-informed, evidence-based care that is person-centred.
What You'll Do:
- You will work alongside an experienced GP in an established pilot outreach program run in collaboration with Central Coast PHN and Point Clare Medical Practice.
- Provide autonomous clinical consultations within the Nurse Practitioner scope—prescribing medications, ordering and interpreting tests, managing referrals, and more.
- Deliver comprehensive health assessments, early interventions, and ongoing clinical support to women and children impacted by DFSV.
- Collaborate closely with GPs, allied health professionals, refuge staff, and community stakeholders.
- Lead initiatives that improve clinical practice, promote health literacy, and advocate for patient wellbeing.
