?Collaborate with Participants & Support Networks – Design and coordinate comprehensive support plans in partnership with individuals, families, and service providers.
?Stay Informed on Industry Best Practices – Keep up to date with evolving standards, innovations, and emerging approaches to deliver effective support strategies.
?Maintain Accurate Health Records & Documentation – Adhere to Action Justice Well-Being’s policies, including the use of ShiftCare for compliance and record management.
?Utilise NDIA PRODA System – Learn and leverage the PRODA system to effectively manage service provisions for participants.
?Support Transition from Custody to Community – Attend Lotus Glen Prison and provide tailored assistance to prisoners with disabilities, ensuring a smooth reintegration process.
?Crisis Management & Risk Mitigation – Develop strategies to handle emergencies and unexpected challenges, ensuring participants receive timely support.
?Financial & Budgeting Guidance – Assist participants in managing their NDIS funding effectively, helping them make informed financial decisions.
? Employment & Education Support – Guide participants in accessing job training, employment opportunities, and educational programs tailored to their needs.
? Advocacy & Rights Protection – Educate participants about their rights and advocate for their needs within the NDIS framework and broader community.
? Technology & Accessibility Support – Help participants access assistive technology and digital tools that enhance their independence and communication.
?Provider & Service Quality Monitoring – Regularly assess the effectiveness of service providers and ensure participants receive high-quality support.
?Cultural & Linguistic Sensitivity – Ensure services are inclusive and respectful of diverse cultural backgrounds and communication needs.
