**About the program**
Consumer Directed Services (CDS) refers to services which are led by and funded by individual consumers. This includes services such as the National Disability Insurance Scheme (NDIS) services, iCare, Insurances and brokers. These services provide support, care and access to services and technology for Australians with disabilities as well as their families and carers.

The Recovery Coach (RC) / Support Coordinator (SC) position involves the coordination and delivery of services purchased through the National Disability Insurance Scheme (NDIS). Responsibilities include managing caseloads, building individual capacity, and maintaining positive relationships with participants.
The primary goal is to assist individuals in achieving the objectives outlined in their NDIS plans, emphasizing personal development and independence. The RC/SC also ensures compliance with NDIS standards, maintains thorough documentation, and may act as an advocate for participants. Overall, the role focuses on providing tailored support to enhance the well-being and goal attainment of NDIS participants.
**Key responsibilities**
- Support participants to understand their NDIS plan
- Support participants to elaborate on their goals and translate these into supports
- Support participants to navigate NDIS service providers and the supports that they provide to achieve their goals
- Support participants to optimise their plan, ensuring they et the most out of their funded supports
- Link participants into community, mainstream and other government services
- Coach participants to manage the coordination of their supports independently
- Work with participants to develop capacity and resilience in their network of supports
- Assist participants to build and maintain a resilient network of formal and informal supports
- Prepare participants for plan reviews (scheduled and unscheduled)

**To be successful in this role, you will need**:
- Certificate IV in preferred course, or above
- Demonstrated experience in working with people living with a disability or mental health condition
- Available to participate in out of hours activities, as required, to deliver on key role requirements and outcomes
- The right to work in Australia and a current, valid Driver's License
- Satisfactory police records check, Working with Children Check (Employee) and NDIS Worker Screening Check
- NDIS Worker Orientation Module Certificate
- COVID-19 Digital Certificate with booster or valid medical exemption
