**About the role - Casual**
Community Health and Inclusion Case Management (CCM) program is funded by Department of Human Services’ Exceptional Needs Unit (ENU). CCM’s purpose is to provide intensive case management to clients referred from ENU.
Under the direction of, and reporting to the Manager, Community Connections Program, the Case Manager will support clients experiencing long-term and/or multi-layered disadvantage in the areas of physical and mental health, economic capacity, etc. Their work will emphasise building client resilience and independence, and connection with relevant supports, including NDIS, and other health services, in line with Baptist Care SA’s Tapa Marnirni-Apinthi Practice Framework.
- Key responsibilities will include, but not limited to:_
- Complete professional intake and assessment of clients.
- Create and manage actionable care plans with clients, integrating information provided at referral and gathered through client assessment.
- Work flexibly with clients, engaging them in adaptive ways and utilising various tools as appropriate, while developing their capacity to engage more broadly.
- Connect clients with purposeful partners

- You will also have the following skills and experience:_
- Bachelor of Social Work (AASW-recognised) or equivalent
- Experience in case management focused service delivery
- Experience with the general principles of risk assessment, and case management more broadly

- A current and valid DHS: Child-Related Employment Screening/ WWCC
- A current and valid National Police Clearance
- Training - Child Safe Environments
- A valid full South Australian drivers’ license (C Class)
- Ability and willingness to travel within South Australia as required
