About the role
The purpose of this role is to work collaboratively and consultatively across multidisciplinary mental health services to actively support and promote discharge planning processes on the ward. This comprises the coordination of key discharge planning and intervention programs including engagement with key stakeholders.
About you
- Provide input, feedback and recommendations as part of the multi-disciplinary treating team and manage the referrals for ongoing treatment for consumers being discharged from the inpatient unit.
- Promote quality discharge practices for the inpatient unit and work collaboratively with the Patient Flow Manager to support patient flow across the service.
- Proactively identify potential barriers to extended length of stay and coordinate care and service planning to support discharge.
- Actively foster a collaborative, cooperative and consultative approach in working with mental health consumers, carers, the multidisciplinary mental health team and with internal and external care providers.

**Mandatory Requirements**:

- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
- Vaccinated against COVID-19 as outlined in the Health Employment Directive NO.12/21and the Queensland Health Human Resources Policy B70.
- This position may be required to work across the various facilities throughout West Moreton Hospital and Health Service and may be required to work shift work including weekends in some rotations.
- This position requires the incumbent to operate a motor vehicle, therefore an appropriate licence endorsement to operate this type of vehicle is required. Proof of this endorsement must be produced before commencement of duties.
