About the role
The purpose of this role is to provide a central point of coordination to work towards broader systemic solutions for long-stay patients with complex clinical needs requiring professional leadership, advocacy, advice and consultation to support and lead multidisciplinary teams to access appropriate and necessary disability or aged care supports, as a part of supporting the continuum of care from hospital to community.
The position will play a high-level strategic role in the development and implementation of rapid response processes, partnerships with external stakeholders and to inform WMH Executive on proposed options for more timely discharge of long stay patients.
About you
**Clinical Practice**:

- Demonstrated advanced level clinical knowledge, expertise and skills in managing complex long-stay patients, and the ability to intergrate this with evidence based theories and referral pathways to residential aged care facilities, disability accomodation services.

**Clinical Knowledge**:

- Demonstrated knowledge of relevant policies and legislation related to long stay patients where there are psychosocial barriers to discharge and requires expertise in NDIS, Disability and Human Rights and providing consultation advice on QCAT and Guardianship administration, elder abuse and domestic and family violence.

**Communication and Team Focus**:

- Demonstrated high level interpersonal skills including the ability to consult and communicate effectively with staff at all levels of the organisation, external organisations and in negotiation to achieve collaborative partnerships for service improvement/enhancement.

**Leadership**:

- Demonstrated ability to provide high level negotiation, consultation, brokerage and advocacy skills to facilitate partnerships and achieve objectives relevant to long stay patients and service flow challenges.

**Quality Focus**:

- Demonstrated ability to identify service level risks and clinical practice risks at a patient and systems level and utilise service data to lead service improvement initiatives to improve patient outcomes and service gaps.

**Mandatory Requirements**:

- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
- Vaccinated against COVID-19 as outlined in the Health Employment Directive NO.12/21and the Queensland Health Human Resources Policy B70.
