An exciting opportunity exists for a Discharge Planning Coordinator to join our dynamic team at Pine Rivers Private Hospital on a full-time basis.

The role of the Discharge Planning Coordinator is to provide assistance to the clinical departments in episode management and the discharge planning process, which should be within the strategic direction of Pine Rivers Private Hospital. The role includes, but is not limited to:

- Acting as an advisor on all matters relating to the discharge planning process and episodic management;
- Knowledge of health fund contracts/Casemix in the private sector;
- Facilitate referrals to day programs via the provision of information to patients and their families, education for staff and treating doctors;
- Provide day programs by ensuring adequate resources are distributed at the hospital;
- Ensure streamline referral process to community and day programs;
- Offer support to patients readmitted to the hospital from community and day programs.

Located on the bank of the South Pine River, our 56 bed mental health facility provides comprehensive inpatient and day programs for our Queensland community. As a leading provider, we specialise in addiction, bipolar and depressive disorders, mood disorders, OCD, PTSD and complex PTSD management, psychosis and chronic pain management. We also deliver specialised therapies in EMDR, TMS and ECT.

Selection Criteria:

- Demonstrated effective communication, interpersonal and team building skills
- Current Registration with the Australian Health Practitioner Regulation Agency (AHPRA) or eligibility for membership with appropriate discipline specific association, such as the Australian Association of Social Workers.
- Commitment to personal and professional development
- Good Interpersonal and time management skills with an ability to work within a multidisciplinary team
- Demonstrated ability to deliver high quality patient care
- Supports and facilitates the patient and family to make informed decisions by providing appropriate information and options regarding health and nursing care
