**What you'll be doing**

The Virtually enhanced Community Care (VeCC) service is a multidisciplinary service that provides both face-to-face and virtual care in the community including care navigation, care coordination and health coaching interventions. The VeCC works with Aboriginal and non-Aboriginal people of all ages living in the Illawarra Shoalhaven district with a life limiting chronic illness, including those requiring acute care following discharge from hospital through the VeCC. The VeCC service supports patients to live independently and comfortably at home as long as possible with access to allied health and nursing staff and remote monitoring of their symptoms and wellbeing.

This position is part of the VeCC multidisciplinary team and is is responsible for the provision of culturally safe, patient centred care coordination services to Aboriginal patients and their families with life limiting chronic diseases.

The Aboriginal Health Practitioner will deliver care through multiple methods including telehealth, virtual communications and face-to-face sessions. The position will work collaboratively with other health staff to provide assessment, care coordination and health education to patients, families and cares that takes into account clinical, psychosocial and cultural needs.

The Aboriginal Health Practitioner will increase Aboriginal people’s access to the VeCC service and end-of-life care with a focus on improving the patient journey for Aboriginal people within their communities. The Aboriginal Health Practitioner will provide a range of services to meet the needs of Aboriginal patients as defined by an agreed Scope of Practice.

The position will maintain the confidentiality of all clients and adhere to all ISLHD policies, practices and guidelines.

**This position is open to Aboriginal and Torres Strait Islander applicants only in accordance with Clause 26 of the Government Sector Employment Rules 2014, applicants must demonstrate Aboriginality in addition to addressing the selection criterion.**

All NSW Health workers are required to have completed an up-to-date course of a COVID-19 vaccine (3 doses for Category A roles, 2 doses for Category B roles) which has been approved or recognised by the Therapeutics Goods Administration (TGA). New applicants must have completed the vaccination course prior to commencement with NSW Health, or provide an approved medical contraindication certificate certifying the worker cannot have any approved COVID-19 vaccines available in NSW.

Acceptable proof of vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement or AIR COVID-19 Digital Certificate. Please provide proof of vaccination if available.

**Selection Criteria**
- Must be of Aboriginal and/or Torres Strait Islander descent (NB applicant's race is a genuine occupational qualification and is authorised by Section 14 of the Anti-Discrimination Act 1977, NSW) and have demonstrated ability to effectively and sensitively liaise and communicate with local Aboriginal and Torres Strait Islander people and communities and agencies.
- Hold a Certificate IV Aboriginal Primary Health Care (Practice),and hold or be able to meet AHPRA standards and complete Registration as an Aboriginal Health Practitioner.
- Demonstrated understanding of the range of factors that have influenced the health status of Aboriginal communities and experience delivering culturally safe health services to Aboriginal communities.
- Demonstrated ability to engage and work autonomously to effectively prioritise workloads under limited supervision within a multidisciplinary team.
