**What you'll be doing**

The Aboriginal Health Practitioner/trainee position within the Assertive Case Management (ACM) program is to work as part of the multi-disciplinary team that provides intensive care coordination to support consumers with chronic and severe substance use issues. The ACM program will target clients with chronic and severe drug and alcohol dependence who experience multiple and complex needs. The ACM program will provide support to assist in the client's stabilisation and reintegration, and where relevant, assist in managing the client's underlying cognitive impairment. The outcomes of the ACM program are to stabilise and reduce drug and alcohol use, reduce preventable hospital presentations for substance use issues, improve access to longer-term treatment and support for clients, improve general physical and mental health, nutrition, and social functioning and reduce the risk of relapse through the establishment of prevention strategies. Within this scope the position will liaise and work with the Aboriginal community and Aboriginal specific service providers to ensure appropriate assessment and referral to drug and alcohol services for specific models and programs, counselling, withdrawal, rehabilitation, group work, opioid treatment services, and education. The Aboriginal Health Practitioner/trainee is responsible for ensuring culturally appropriate treatment and community prevention strategies are conducted in consultation with staff and consumers of the service.

**Selection Criteria**
- Must be of Aboriginal and/or Torres Strait Islander descent (NB applicant's race is a genuine occupational qualification and is authorised by Section 14 of the Anti-Discrimination Act 1977, NSW). Must have a demonstrated ability to effectively and sensitively liaise and communicate with local Aboriginal and Torres Strait Islander people, communities, and agencies.
- Hold or be willing to work towards a Certificate IV in Aboriginal Primary Health Care (Practice), and eligibility to be registered with the Australian Health Practitioner Regulation Agency (AHPRA).
- Demonstrated ability to engage and work autonomously to effectively prioritise workloads under limited supervision within a multidisciplinary team.
- Demonstrated skills in client assessment, support, referral and advocacy in drug and alcohol or related field and a commitment to quality improvement.
