About You

**Additional attributes we are looking for include**:
Previous experience in assertive outreach, youth work, case management, mental health, justice or AOD (professional, volunteer, lived experience); or qualifications in psychology, mental health, social work, criminology or other relevant fields
The ability to effectively and quickly build rapport with people and think outside-the-box to achieve positive outcomes
Flexibility, patience, confidence, initiative and emotional resilience
Effective listening and negotiation skills & a strong but adaptable communication style with people of diverse backgrounds and abilities
The ability to work autonomously to manage your case load
It is a requirement of this position to be fully vaccinated against COVID-19 and meet all compliance requirements in relation to NDIS Work Screening, Working With Children’s Check, work rights and possess a current & valid driver’s license.
About the Role
ermha365 provides Early Intervention Psychosocial Support Response (EIPSR) program in partnership with Forensicare at Thomas Embling Hospital in a forensic and community setting. EIPSR provides specialist psychosocial support to help eligible people to build their capacity to better manage their mental illness, develop practical life skills for independent living and social connectedness, and if eligible, transition to the NDIS.
The Support Facilitator’s goal is to enable rather than do, to promote independence, development and confidence of the individuals.
**The Support Facilitator role entails**:
- Provide psychosocial support to people with a severe mental illness in a forensic setting
- Working with the individual, (their family, if relevant) and their clinician and other supports to access and engage with a range of supports
- Develop recovery goals, implement steps to achieve set goals; monitor and track the progress of goals
- Work closely with clinical staff, collaborate with care teams and support planning and review, direct support and exit planning
- Facilitation of recovery groups in a forensic setting, and outreach support to peoples’ homes or in the community
Program hours are 8:30am-5:00pm Monday to Friday. This is a contract position ending 31/12/2023 (potential for extension).
