Care Manager

**The role**: The full time Care Manager will be the primary contact for residents, their family members, and representatives, and will respond to and resolve feedback, care needs and clinical incidents, for residents living at ViTA in Daw Park.
In partnership with the Home Manager - ViTA, this role will be valuable contributor and a member of the high performing leadership team responsible for ensuring that the Residential home meets all compliance obligations, as well as achieves business and service objectives.
**What you'll do**:
- Complete comprehensive and accurate nursing assessment for resident
- Ensuring the care plan is holistic and person-centred to meet the needs of the resident
- Support the utmost wellbeing of residents
- Consult and communicate with a multi-disciplinary team internally and externally to identify resident care needs, plan interventions and review outcomes of care
**About You**
- Minimum 2 years post-graduate nursing experience.
- Ability to maintain appropriate standards and systems to the highest quality
- Knowledge and understanding of the Standards of Aged Care, legislation, and regulations.
- Excellent attention to detail
- High level of clinical competence
**You will need**
- A nursing qualification and current registered nurse registration with AHPRA
- To care about supporting older people to live a Good Life.
- Current Flu & COVID-19 vaccinations (3)
- A satisfactory Police Check for working with vulnerable groups less than 12 months at commencement
