About the Role

A highly rewarding opportunity has emerged for a seasoned Dementia Consultant to join HammondCare at our Daw Park site on a Fixed Term Full-Time Contract until June 2026. This pivotal role is instrumental in delivering high-quality dementia support services to individuals and their carers, while also mentoring and modelling best practices among aged care staff.

Key Responsibilities

  • Provide expert consulting and advisory services in dementia care, both within and outside of HammondCare.
  • Develop the skills and expertise of aged care staff and carers to effectively support individuals living with dementia.
  • Develop and implement personalized advice, recommendations, and strategies to address challenging behaviors related to dementia.
  • Assess clients with dementia-related behaviors and provide tailored resources and suggestions for effective care provision.
  • Stay up-to-date with current evidence-based guidance and ensure alignment with the Dementia Centre's standards.
  • Plan and execute engagement initiatives with individuals and teams to achieve agreed practice development objectives and quality improvements.
  • Support other projects and assignments as required, leveraging your areas of skill, expertise, and knowledge.
  • Familiarize yourself with HammondCare's Mission in Action, Model of Care, Case Management Framework, and practice guidelines, policies, and procedures.

Requirements

  • Proven expertise in supporting individuals living with dementia.
  • Experience working in Australian residential aged care and/or community aged care settings.
  • Excellent negotiation, communication, and relationship-building skills.
  • Strong problem-solving and decision-making abilities.
  • Ability to prioritize multiple tasks and meet deadlines.
  • Superior written and verbal communication skills.
  • Effective stakeholder management.
  • A valid Australian driver's license and willingness to travel.
  • Empathy and patience.
  • A relevant qualification in Allied Health, Nursing, Aged Care, or Mental Health is essential.
