The Trafficking and Slavery Safe House works collaboratively with a person-centred, trauma-informed, and holistic approach to service delivery to ensure the best outcome for all who seek our assistance. The position will work collaboratively with the Safe House team, Salvos Housing and partner agencies to achieve positive client outcomes and provide a comprehensive client-centred service within the values of The Salvation Army.

About the role

The Case Manager (Tenancy, Community, and Independence) will provide holistic and client-centred casework support focused on independent living skills, community connection and tenancy management to participants on the Modern Slavery Transitional Housing Program, and where relevant other Safe House clients. This includes referral, program intake and assessment, case planning and goal setting with the aim of supporting participants to maintain their tenancy, make local community connections, gain rights education and increase their independent living skills.

This is a Max-Term Part-Time opportunity (19 hours per week) until 1 December 2023. The role is based in Surry Hills NSW. Salary and conditions are in accordance with the SCHADS Level 5 Award.

You will successfully- Oversee Transitional Housing Program referrals by conducting assessments to determine eligibility, manage waitlist, ascertain appropriate alternative supports and make referrals as needed.- Manage caseloads as allocated by the Program Manager by building effective relationships with clients, that encourage self-determination and participation.- Provide case management support to clients in accordance with TSA case management models and standards of practice to ensure appropriate assessment of the client’s issues, needs and circumstances from initial referral and intake to exit.- Prepare, develop, and implement individual case plans in collaboration with clients and include a range of interventions that address the needs or goals of clients, by monitoring progress and outcomes of individual case plans to ensure the plan continues to meet client needs or goals.- Identify and assess risks to clients, staff and TSA and ensure service gaps are identified and recommendations for service delivery improvements are reported to the Program Manager.- Participate in rostered on-call duties for Trafficking and Slavery Safe House residents in a shared capacity between team members.

You will have- A Degree qualification in social work, welfare, community development or related field OR demonstrated high level of sector knowledge and skills attained through previous appointments, service, and study/development.- A current NSW Working With Children Check- The willingness to undergo a National Police Check- Considerable relevant experience in a social service environment.
