We offer a range of services including Harm Reduction and Primary Health Services as well as a range of treatment types (including residential treatment centres, community-based support programs, counselling and withdrawal management services). About the role

We are currently seeking a values-driven and proactive Case Manager, reporting to the Program Manager. The Case Manager for Oasis Drug and Alcohol Youth Support will provide case management to young people aged between 12-24 within the South Eastern Sydney Health District. The Case Manager will provide early crisis and long-term interventions through a flexible case management model underpinned by trauma informed care. The Case Manager will work towards reducing alcohol and other drug use related harm by delivering a client centred, high quality and safe case management and care coordination approach. The Case Manager will improve the health, well-being and social outcomes of clients by delivering comprehensive alcohol and other drug support interventions.

You will successfully
- Provide case management and care coordination for up to 25 Young People (12-24) for up to 6 months
- Provide accessible information to clients, family, and other service providers about the service
- Assess clients with a range of co-existing physical health, mental health, and social conditions
- Facilitate regular contact and support for clients who are on a wait list
- Maintain minimum weekly direct contact with all clients
- Develop effective and shared care relationships with other health and social service providers
- Facilitate client’s access to alcohol and other drug treatment services
- Establish the scope of services and/or resource supports for clients that may be accessed with brokerage
- Participate in individual client case review and planning meetings with other providers every 6 weeks
- Evidence work with clear, concise recording and documentation in 48hrs, upholding confidentiality always
- Demonstrate critical thinking and reflective practice in all case work, documentation, and supervision
- Adhere to mandatory reporting legislation and guidelines
- Participate in team meetings, supervision, training and build positive partnerships with key stakeholders
- Support other members of team (Case Manager, Counsellor, Group Facilitator) to achieve shared goals
- Provide culturally responsive support to diverse groups through work
- Work collaboratively with co-workers, other Oasis programs and external agencies

This is a permanent full-time position in Surry Hills, NSW with an annualised salary range of $77,577 - $83,486 per annum + super.

You will have
- Minimum Tertiary Qualifications in Social Work or similar discipline, preferably Bachelor’s Degree or Higher (other relevant qualifications/experience within Community Services, Social Work or similar discipline will be considered)
- Previous experience in case work or similar role, providing case management to vulnerable young people from diverse backgrounds
- Valid Employee NSW Working With Children’s Check
- Current NSW Driver’s Licence
- Proof of COVID-19 Vaccination or Exemption
- Eligibility for registration with peak body (where applicable)
- A willingness to undergo a National Police check
