The Chronic Pelvic pain (Connect) Clinical Nurse Consultant (CNC) is an experienced practitioner who uses evidence-based practice in the nursing management of chronic pelvic pain patients to optimise their health outcomes.

The CPP CNC coordinates the CPP clinic, working collaboratively with the multidisciplinary team to ensure positive patient outcomes. Providing consultation, assessment, and care planning to assist patient in making informed choice about their ongoing healthcare needs.

The CPPC program is a new program funded through the elective surgery reform project. The aim is a MDT approach to aid patients on long wait list for surgical intervention. To provide new stratergies in management of their pain.

**About the role**

**Lead the CPP clinic appointments face to face or via telehealth for the following**:

- Assess the need for referral for NESB patients.
- Medication review and Progress review
- Pain questionnaire assistance
- Pain management education

**Multidisciplinary team care coordination**:

- Documentation of patient outcomes following MDT appointments
- Inter hospital communication including requesting reports, pathology, imagining and procedure referrals.
- Liaise with GP
- Monitor DNA/ referral closure documentation.
- ePPOC attendance documentation

**Triage**:

- Triage referrals to the chronic pain clinic, chronic pain connect clinic and the chronic pain intervention (WEC) clinic.
- Ensure compliance of referral guidelines and access policy.
