**About you**

We are looking for a dedicated and passionate School Support Clinician to be a part of our intake team. This role works closely with schools in the City of Knox and Shire of Yarra Ranges to provide support services to youth who are experiencing mental health challenges. As part of the intake team your role will be to serve as the primary contact for referrals and other youth mental health related activities. This position manages a case load and covers the process from comprehensive risk assessments with young people and their families to referrals with other services. An integral aspect of this role is monitoring and advocating for young people, and ensuring collaboration with a multi-disciplinary team, consisting of GPs, psychologists, nurses and other allied health workers are engaged in the process where required.

**Key responsibilities**
- Work closely with the Schools in the City of Knox and Shire of Yarra Ranges
- Conduct intake assessments, Mental State Examinations and comprehensive risk assessments with young people and their families from selected schools
- Present assessments at Intake Meetings and case consultations. Formulate referral and recovery plans for referral for young people as appropriate. Actively monitor young people, providing follow up if non-attendance to scheduled appointments or to provide follow up to support implementation of care plan goals and objectives
- Advocate on behalf of the young person with other workers, schools and employers as required
- Coordinate the referral of young people to services within headspace Knox or Lilydale or to other relevant services to ensure a coordination of care for the individual’s needs.
- Actively monitor young people, providing follow up if non-attendance to scheduled appointments or to provide follow up to support implementation of care plan goals and objectives
- Develop and facilitate groups where required.

**Qualifications and skills**
- Holds Qualifications and registration in Psychology, Social Work, or occupational Therapy.
- Completion of a Criminal History Check and Employee Working With Children Check (or State equivalent) prior to commencement of employment and as required by legislation and policy during employment, as well as a duty to disclose relevant information that may arise after employment has commenced
- Current state-based driver’s license
- Highly developed clinical skills, including the ability to assess, treat and manage mental health challenges in young people
- Highly developed interpersonal, verbal and written communication skills.
- Highly developed presentation skills.
- Exceptional organisational and time management skills.
- Ability to work independently and cooperatively in a team environment.
