**Position**:
We are currently seeking a highly skilled and compassionate Support Coordinator to join our dedicated team. As a Support Coordinator, you will play a key role in coordinating and overseeing the provision of specialised support services to our clients with varying needs.
**Responsibilities**:
- Coordinate and manage the delivery of specialized support services to clients with complex needs, ensuring the highest quality of care is provided.
- Conduct comprehensive assessments to identify clients' individual needs, preferences, and goals.
- Develop and implement personalized support plans in collaboration with clients, their families, and a multidisciplinary team.
- Monitor and evaluate the effectiveness of support plans and make necessary adjustments to ensure optimal outcomes for clients.
- Provide ongoing guidance, and support to other support coordinators, ensuring they have the necessary knowledge and skills to deliver specialised care.
- Act as a liaison between clients, families, support workers, and external stakeholders, maintaining clear and effective communication channels.
- Ensure compliance with relevant regulations, policies, and procedures, including safeguarding, confidentiality and health and safety guidelines.
- Stay up-to-date with industry best practices and developments, continuously enhancing your knowledge and skills.
**Qualifications and Requirements**:
- Minimum Diploma level in relevant field such as Social Work, Psychology, Counselling etc.
- Proven experience in coordinating and delivering support services to individuals with complex needs.
- In-depth knowledge of person-centred approaches and best practices in care and support.
- Excellent assessment, planning, and organizational skills.
- Strong communication and interpersonal skills, with the ability to build effective relationships with clients, families, and colleagues.
- Ability to work collaboratively in a multidisciplinary team and adapt to changing priorities.
- Empathy, compassion, and a genuine commitment to enhancing the lives of individuals with diverse needs.
- Knowledge of relevant legislation, regulations, and quality standards.
- Valid driver's license and access to reliable transportation.
