**We are looking for**
- Tertiary qualifications or equivalent in a relevant discipline - nursing (CNS or above) or allied health.
- Highly developed interpersonal and communication / presentation skills including report writing skills
- Demonstrated experience of word processing, database and spreadsheet computer packages including Microsoft Office and Excel.
- Current Working with Children Check is required for this role.

**About us**

The Agency for Clinical Innovation (ACI) leads innovation in clinical care across NSW. We do this by bringing clinicians, patients and healthcare managers together to design and implement new ways to deliver healthcare.

**What you will be doing**

Covering long service leave for 12 weeks, the Transition Care Coordinator will work with young people and clinicians to coordinate their transition from paediatric to adult health services.

The Transition Care Network (for young people with a chronic illness/disability) was established in 2004. A Network Manager, three Transition Care Coordinators (TCCs) plus 3 part time support positions were appointed with the primary aims to:

- Improve continuity of care for young people transitioning from paediatric to adult health services
- Ensure adequate and continued follow up in the adult health care system
- Improve support offered during the transfer of care

The primary purpose of the Transition Care Coordinator role is to:

- Facilitate coordination of transition for young people with chronic conditions (in line with referral criteria) throughout the Western Region in collaboration with clinicians;
- Create networks and pathways that facilitate a smooth and effective transition
- Act as a resource person for clinicians around individual case management
- Collect and evaluate data around transition
- Identify service gaps and escalate through Network Manager and other appropriate channels;
- Develop transition plans
- Support research around Transition Care
- Be a strong advocate for young people with chronic conditions and their families during the transition to adult health services
- Assist young people and their families to adjust to a new adult team and service, to ensure engagement.

**Selection Criteria / Questions**
- Experience in the health system in a clinical position relating to complex and chronic illness which originates in childhood and / or adolescence.
- Demonstrated experience working in a multi-disciplinary team
- Demonstrated effective time management skills and ability to work with mínimal supervision and to facilitate change within a complex environment.
- Demonstrated ability to engage clinicians from a variety of backgrounds with differing priorities and facilitate the creation of networks.
- Current unrestricted driver's license

This position is a temporary position and **requires full working rights in Australia **(e.g. Australian citizenship/ permanent resident). **COVID-19 Vaccination Compliancy**

All NSW Health workers are required to have completed a primary course of a COVID-19 vaccine which has been approved or recognised by the Therapeutics Goods Administration (TGA). Additionally, Category A workers are required to receive a booster dose three months after completing the primary course of COVID-19 vaccinations. New applicants must have completed the vaccination course prior to commencement with NSW Health, or provide an approved medical contraindication certificate (IM011 immunisation medical exemption form) certifying the worker cannot have any approved COVID-19 vaccines available in NSW.
