**About You**
- You have a degree qualifications in health or health-related discipline and extensive professional experience in Community Aged Care and Chronic Disease Programs.
- You are passionate and share our commitment to providing compassionate and effective care.
- You are collaborative and work well with a diverse team of healthcare professionals.
- You have proven experience in aged care service delivery management.

**Where You'll be Working**

In this role you will be based across The Central Coast.

**Key Responsibilities of the role include**:

- Lead and manage aged care program service delivery, ensuring alignment with organizational objectives and client needs.
- Oversee operational priorities, set service delivery goals, and implement quality improvement initiatives.
- Manage human, physical, and financial resources to deliver exceptional care outcomes.
- Improve clinical data reporting and compliance with regulatory requirements.
- Develop and manage service level agreements with key stakeholders.
- Collaborate with leadership and stakeholders to develop and implement aged care programs.
- Facilitate change management processes and build a positive workplace culture.
- Develop and maintain strong partnerships with internal and external stakeholders.
