Home Care Package Case Manager

We are looking for a compassionate and energetic Care Manager to work with our Home Care Package clients. As a Home Care Package Case Manager, you will work with clients, family members and representatives to provide quality service and to ensure clients get the services they need to remain in their homes safely, with dignity and choice.
**The Role**
We are seeking a Home Care Package Case Manager to assist existing clients, and onboard and manage new recipients, in the north and southern suburbs of Melbourne. Your case load will be mixed including level 1 to 4 package recipients with a variety of care needs. The role reports to the Home Care Package Manager.
We are looking for Care Managers who are compassionate and empathetic.
- You must be capable of building positive and effective partnerships with clients, their representatives, and other stakeholders including clinical experts, allied health professionals as well as support workers.
- Use your skills and knowledge of care requirements to conduct assessments, and develop consumer directed care plans focused on fostering client independence, wellness and reablement.
- Ability to recognise and respond to client clinical care requirements and health and wellbeing feedback and manage client issues and outcomes.
- Work collaboratively with other team members to ensure that our program is best practice and meets the Aged Care Quality Standards.

**Qualifications**
- 1-3 years of industry experience or a related field of experience preferred with transferable skills from case management or clinical care in other environments.
- Ability to communicate effectively with people from diverse backgrounds both in a verbal and written capacity.
- Building positive and effective partnerships with clients, their representatives, and other stakeholders including clinical experts, allied health professionals as well as support workers
- A current driver's license is essential to the role
- Excellent customer service and problem-solving skills
- High quality organizational skills
- The ability to work well under deadlines and to multitask
- The ability to build relationships and coalitions within the community
- Excellent verbal and written communication skills
- Excellent critical thinking and problem-solving skills
- A police check is also mandatory.
- Our vaccination policy requires a minimum of two COVID vaccinations.
