**Care Consultant**
- **Full Time position**

BodeWell is looking for an experienced and motivated Care Consultant to join. The role is instrumental in providing support and advice to clients to assist them with their well-being goals.

In 2005, Arcare started offering in-home services and support to the community through our home care division. Our home care team can provide privately funded in-home services, government funded home care packages and disability services across Metropolitan Melbourne, the Mornington Peninsula, the Sunshine Coast, Brisbane and the Gold Coast.

**About the Role**

We are now looking for an experienced and motivated Care Consultant to join our Home Care Packages Team to help support and advise clients in the South East region. The role is instrumental in providing support and advice to Home Care Packages clients to assist them to keep living at home.

**Skills and Experience**
- Tertiary qualification e.g. Bachelor in Welfare or Human Services, Social Work, Nursing or Allied Health
- Experience and knowledge of Disability and Aged care sectors
- Experience in working with Home Care Packages clients (desirable)
- Knowledge of Carelink + would be an advantage (desirable)
- Proven ability to build positive long-term relationships with clients, customers, allied health professionals
- Experience in achieving and maintaining stable client programs, whilst helping clients to get the best possible value from their funding
- Ability to work autonomously with strong communication and reporting skills
- Be able to actively identify and realise business growth opportunities.
- Effective communication skills, reliable, flexible, and a team player
- Current Police Clearance and Statutory Declaration
- Current Drivers Licence and own vehicle

**Key Responsibilities**
- Building positive long-term relationships with clients, customers, allied health professionals
- Achieving and maintaining stable client programs, whilst helping clients to get the best possible value from their funding
- Effective communication and team work with Service coordination staff
- Developing and maintaining Care Plan requirements for clients
- Expectation Management of clients and key stakeholders
- Ensuring the safety of the Support Workers working with the identified client group
- Actively identify and realise business growth opportunities.
