**Clinical Care Consultant | Bodewell VIC | Home Care Packages**
Bodewell is seeking a Clinical Care Consultant to provide high-quality person-centered care management and clinical assessment for home care clients in VIC. Reporting to the State Manager Home Care & Compliance, you will ensure the safe provision of clinical care, compliance with regulations, and building positive relationships with clients and stakeholders.

**About the Role**
As a Clinical Care Consultant, you will play a crucial role in managing client care and well-being, including specialized areas such as continence management, conducting clinical assessments and intervention, medication management, dementia care support, and diabetes management, wound management, and education. Your responsibilities include assessing, implementing, and evaluating clinical interventions in a timely manner, and ensuring the safe provision of clinical care in home care settings. You will also collaborate with external providers and maintain compliance with relevant legislation and standards.
**Key Responsibilities Include**
- Conducting clinical assessments and interventions, including specialized areas such as wound management, continence management, medication management, dementia care support, and diabetes management and education.
- Initiating service agreements with external providers when necessary.
- Demonstrating comprehensive knowledge of industry regulations and best practices.
- Efficiently managing client care within Bodewell policies and procedures.
- Mentoring and supporting direct care staff.
- Responding to clinical inquiries and working with health professionals to develop care plans.
- Monitoring and reviewing care plans within budget limitations.
- Advocating for consumers to ensure timely clinical care.
- Coordinating Peer Review Meetings and follow-ups.
- Networking and building relationships with community resources.
- Ensuring compliance with legislation, guidelines, and standards.
- Maintaining consumer confidentiality and privacy.
- Participating in quality reporting activities.

**Qualification**
- Tertiary qualification in Nursing
- Hold and maintain current AHPRA RN registration
with the Nursing and Midwifery Board of Australia
- NDIS Worker Screening Check
- Right to Work in Australia
- WWCC (VIC Only)
- Current State Driver’s License
- Computer literacy - including Microsoft Office, Internet and Database Systems, CL+
- Current required vaccination
- Knowledge of the Home CarePackage Program
- Experience in Community Nursing
