**About the role**:
Kalyra's Community Coordinators play a vital role in the implementation and ongoing support to older South Australian's living in the local community. Our focus is to ensure the people we support can remain as independent as possible and live the life they want to live. This role is supported by teamwork You'll work closely with our Program Assistants and Home Support Workers to take a collaborative approach to the way we deliver care by taking a do with, not for approach.

**About you**:
You have confidence in developing client support plans and associated budgets that enhance the clients independence. You will have strong communication skills and be able to effectively communicate with allied health professionals, family members and the Kalyra team. You will also be a supportive leader who works collaboratively with our support team to achieve the best possible outcomes for our clients. You'll also need:

- A certificate III in Aged Care or a higher qualification in nursing/community services or have extensive experience.
- You will also hold a National Police Check or be willing to obtain one before commencing employment.
- Covid vaccinations x 3 (including at least one booster) and a 2023 flu vaccination
- Driver's licence
- Ability to work on an on-call roster (2-3 times a year or when required)
