The Role
Reporting to the Team Leader, we require a permanent full-time Community Coordinator (CHSP) (76 hours per fortnight Monday to Friday) to assist with the implementation and ongoing support to elderly people living at home. It’s all about the client and you’ll work closely with our Program Assistants and Home Support Workers to ensure we are meeting our clients needs in a way they would like. This is a full time role, based out of Darlington. You will enable the delivery of high quality services to clients, through the analysis and problem solving of customer needs, effective stakeholder engagement, financial management and coordination of home support worker team members.
About You
**To succeed in this role, you will possess the following**:
- Certificate IV in Aged Care or a higher qualification in Nursing/Community Services or extensive experience
- Outstanding communication skills and the ability to build effective and positive relationships with clients and team members
- The ability to support, mentor and lead a team effectively to achieve client goals and be able to problem solve and work through challenges as they arise
- A passion for helping older people retain their independence
- Hold a valid National Police Check or be willing to obtain one
- Current Driver's License
- Fully vaccinated for Covid-19 (minimum 3 vaccinations) and influenza
