**About us**
Our
**Logan** Community Aged & Disability team is key in providing care to a range of clients, including older people and those with disabilities. Our teams provide support and services that enable our clients to maintain their independence - living happily and healthily within their own homes.
**About the role -**
As a Client Liaison Officer you will undertake assessments, support planning, packaged care coordination and case management activities designed in collaboration with the client to assist the client in achieving their identified goals. Support the client to manage and monitor their budget to ensure care packages are financially sustainable and monitor the services provided to ensure these are following relevant service models and guidelines.

You will have a background in Community Services, Aged Care or Disabilities.
- Certificate IV in Health, Community Services, Aged Care or Disability Support
- Proven understanding of assessments, packaged care and case management
- Ability to engage clients and work effectively with them to achieve identified goals
- Support and guidance to clients with budget management and financial sustainability
- Exceptional communication skills
- Ability to advocate strongly for your clients
- A sound understanding of the community care system
- A current National Police Clearance
- Current Covid Vaccinations
