Working closely together in a multidisciplinary team, you will collaboratively support an older person living in the community with service options to enhance their independence, control, wellbeing and quality of life in the role based from the Christie Downs Community Hub.

**What you'll do**:

- Support customers with understanding, developing and reviewing their Home Care Package
- Conduct assessments, develop support plans, undertake reviews, implement and facilitate clinical and care interventions
- Promoting ACH Groups Services and Solutions
- Liaise effectively with external stakeholders, including carers, acute, respite and other community services, medical and allied health professionals
- Provide robust budget management, ensuring that accurate budgets are maintained so the customer can be supported as required

**About You**

You want to make the most of your expertise, do what you do best and thrive on the challenge of providing our customers with quality care.
- Knowledge of 'Consumer Directed Care'
- Ability to manage budgets
- Ability to assist individual requirements, including customers with a range of complex needs
- A flexible approach to your work with the ability to prioritise and multi-task
- Empathy, understanding, high degree of integrity and diplomacy
- Excellent interpersonal, communication, negotiation, and problem-solving skills

**You will need**
- A Diploma in Community Services Coordination, Diploma / Certificate IV in Community Services, or similar equivalent qualification
- OR a Nursing or Allied Health qualification and current registration with AHPRA qualified Enrolled / Registered Nurse or Allied Health Professional
- To care about supporting older people to live a Good Life.
- Flu & COVID-19 vaccinations (including booster, if eligible)

**It would also be helpful if you had**:

- A **CURRENT **Police Check
- Driver's license
