Western Health is seeking a suitably qualified, highly motivated and dynamic Care Coordinator (nursing or allied health background) to join our multidisciplinary team at Dame Phyllis Frost Centre (DPFC), a maximum-security women’s prison, in a full time, ongoing capacity.
The Care Coordinator is responsible for coordinating patient assessments from the point of reception through to discharge planning. The role will also facilitate the development of Integrated Care Plans for those patients identified as having complex health needs. The Care Coordinator works within a multidisciplinary team model of care with other Western Health teams, and also has a role liaising with other external service providers onsite, such as Forensicare. The Care Coordinator will contribute to team based care plans developed in collaboration with the patient; and the subsequent documentation of these interventions and plans.
They will also play a vital role in ensuring that patients are actively engaged with their GP or other community health supports especially on discharge.
As our Care Coodinator, you will be part of a well supported and innovative team and you will be onboarded with orientation and training to the custodial setting, including additional training to support the delivery of culturally appropriate, trauma informed, gender affirming care for all patients, including priority cohorts. This role is both operationally and clinically supported by Western Health staff.
This role presents a significant opportunity for both clinicians seeking to gain experience in custodial settings as part of their career development, as well as for clinicians committed working with marginalised and vulnerable cohorts.
