Job Summary

We are seeking an experienced Home Care Coordinator to join our team in providing exceptional care and support to our clients. As a key member of our team, you will be responsible for coordinating and delivering high-quality home care services that meet the unique needs of each client.

About You

To be successful in this role, you will require:

  • A Certificate III in Individual Care/Aged Care or higher qualification, such as Nursing;
  • A current driver's license;
  • Competence in using IT software or willingness to learn;

You must also have excellent communication and interpersonal skills, with the ability to work collaboratively with clients, families, and other healthcare professionals.

Key Responsibilities

The Home Care Coordinator will be responsible for:

  • Coordinating and delivering home care services, including personal and home care duties;
  • Developing and implementing individualized care plans that meet the unique needs of each client;
  • Maintaining accurate clinical documentation and records;
  • Communicating effectively with clients, families, and other healthcare professionals;
  • Ensuring compliance with relevant legislation, policies, and procedures;
Working Arrangements

This is a full-time position working approximately 38 hours per week. You will be required to work flexibly, including some evening and weekend shifts, to meet the needs of our clients.
