The Manager Heathy Communities will provide effective leadership and management to North in line with NRCH values and strategic directions, including the principles of population health, health promotion, service Integration, community engagement and empowerment and health outcome evaluation.

As part of the Management team within their program portfolio, the Health Communities Manager will contribute to the strategic development and effective operations of the services within the overall Community Health context, ensuring services adhere to funding and policy guidelines. This will include working in partnership with other departments and external stakeholders develop, review and implement strategies and plans that improve social inclusion, equity and access to health services and systems.

The Manager Healthy Communities will ensure service delivery key performance indicators and targets are met as per agreements with funding bodies and/or program workplans whilst ensuring the budget is managed efficiently and resource allocation matches the needs of the service and community.

The Manager Health Communities will identify and monitor risks, drive quality improvements and ensure the services are efficient and meet community and client needs, providing an exceptional consumer experience that improves measurable outcomes.
- The role will require participation in network committees and meetings to build strong stakeholder partnerships and will involve regular reporting to relevant organisation committees and executive, funding agencies and external stakeholders. The role will include seeking grant funding for aligned projects and service development opportunities_

**Fixed term 1.0 **_FTE_** until 24/5/2024**

**Roles and responsibilities include, but are not limited to**:

- Establish, manage and evaluate a Health Communities Strategy for NRCH, including specific Health Promotion Plans that work towards becoming a health promoting health service.
- Work collaboratively to ensure relationships with key stakeholders supports NRCH Healthy Communities Strategies.
- Initiate and maintain positive relationships with key partners to drive service integration to improve client experience and health outcomes.
- Provide advice and expert opinion to the CEO and Executive in population health, that will inform strategy and operational planning to ensure resources are allocated to meet identified community need.
- Plan, develop and report on Health Promotion Plans in line with funding and service agreements to meet required timeframes.
- Meeting funding agreement targets and workplan service levels.

**Key Selection Criteria**

**Essential qualifications, attributes and skills include, but are not limited to**:

- Tertiary Qualification (Diploma level or above) in Health Promotion or Public Health, or equivalent

Please note it is a requirement for all employees of North Richmond Community Health to demonstrate evidence of mandatory immunisations/vaccinations prior to commencement of employment, based on the Immunisation Category their position falls under.
