**About the Role**:
We are seeking a dedicated and proactive Support Coordinator to join our team at Farris Care Services. This full-time position is available immediately and offers a competitive salary starting from $75,000 per year.

As a Support Coordinator, you will play an integral role in assisting NDIS participants in navigating and implementing their NDIS plans. Your primary responsibility will be to support participants in establishing the necessary supports and helping them build their capacity to manage their supports independently over time.

**Main Duties and Responsibilities**:

- **Participant Direct Support**:Assist NDIS Participants in implementing their plans and increasing their capacity to manage and direct their own supports and maximise the value of received supports by participants
- **Plan Implementation**: Action referrals promptly to initiate support services. Support participants in assessing various mainstream, informal and provider options and aid in the selection of preferred options or providers.
- **Documentation, and Record-Keeping**: Maintain accurate and detailed records of participant interactions, progress and outcomes.
- **Community Linkages, Key Contact, and Liaison**: Act as the primary point of contact for service related matters, complaints, major changes, and plan reviews. Establish links to mainstream or community services such as housing, education, transport, and health.
- **Problem Solving and Coordination**:Provide participants with assistance to resolve issues or challenges arising from service agreements and ensure participants understand their responsibilities under service agreements.
- **Community Engagement**: Arrange business meetings with prospective participants and participant groups. Promote the organisations services addressing or predicting participant objectives.

**Qualifications**
- A relevant degree in fields such as social work, psychology, disability studies, community services or a related field.
- Current WWCC, NDIS Screening Check, First Aid Certificate and Police Check.
- Ability to understand the NDIS Price Guide and flexibility within budgets, the NDIS legislation and rules, including provisions relating to a reasonable and necessary supports and the role of the mainstream service systems.

**Skills and Experience**
- **Case Management Experience**:Experience in case management or coordination is valuable for navigating complex support plans and leasing with various stakeholders.
- **Disability Sector Experience**: Previous experience working within the disability sector, whether as a support worker, case manager, or in a related role, is essential.
- **Client-Focused Approach**: Must be able to work directly with clients or participants, working within their individual capacity for understanding and adjusting approach accordingly, to assist them in tailoring their goals, preferences, needs, and support plans for the best possible outcomes.
- **Communication Skills**: Strong communication skills are vital for building rapport with participants, their families, support workers, and other stakeholders. Clear and empathetic communication helps in understanding and addressing participant needs.
- **Problem-Solving Abilities**: NDIS Support Coordinators often need to find creative solutions to challenges that arise in implementing support plans and achieving participant goals.
- **Team Collaboration**: Experience collaborating with multidisciplinary teams, such as therapists, medical professionals, and service providers, ensures effective coordination and holistic support for participants.
- **Regulatory Knowledge**: Familiarity with relevant industry regulations, codes of practice, and standards ensures that support services are delivered in a compliant and ethical manner.
- **Cultural Sensitivity**: Experience working with diverse cultural backgrounds and understanding the unique needs and preferences of participants from various cultural contexts is valuable.
- **Technology Proficiency**: Proficiency in using relevant software for documentation, communication, and case management can streamline administrative tasks and enhance efficiency.
