**Job description**

As an NDIS-approved provider, they are seeking a dedicated and enthusiastic Support Coordinator to join their team.

As a NDIS Support Coordinator, you will play a crucial role in assisting individuals with disabilities to navigate the National Disability Insurance Scheme (NDIS). You will work closely with participants to understand their needs, preferences, and aspirations, and then collaborate with them to develop and implement a comprehensive NDIS plan. This role requires strong communication skills, empathy, and the ability to coordinate services from a variety of providers to ensure the best possible outcomes for our clients. With the ability to work from home up to 3 days per week.

**Key Responsibilities**:

- Engage with NDIS participants to understand their goals, needs, and preferences.
- Collaborate with participants to develop and implement individualised NDIS plans.
- Assist participants in accessing appropriate supports and services based on their goals.
- Coordinate services from various providers to ensure seamless service delivery.
- Provide guidance and information to participants about available NDIS funding and options.
- Maintain accurate and up-to-date records in accordance with NDIS guidelines.
- Monitor the progress of participants' goals and make necessary adjustments to plans.
- Advocate for participants and ensure their voices are heard in the NDIS process.
- Stay updated on NDIS policies, procedures, and changes to provide accurate information to participants.

**Qualifications and Skills**:

- Degree in a relevant field such as Social Work, Psychology, Disability Studies, or related disciplines (preferred but not mandatory).
- Strong understanding of the National Disability Insurance Scheme and its principles.
- Excellent communication and interpersonal skills.
- Empathetic and patient approach when working with individuals with disabilities.
- Ability to work independently and within a team.
- Exceptional organisational and time management skills.
- Proficiency in record keeping and documentation.
- Knowledge of local service providers and community resources is a plus.
- Previous experience as an NDIS Support Coordinator will be highly regarded.

**Salary**: $59,629.69 - $85,000.00 per year
