Job Summary

The RN Case Manager facilitates development, implementation, coordination, and evaluation of individualized patient care plans. Collaborating with patients, families, physicians, and the interdisciplinary team, the role ensures seamless care from acute to post-acute settings. The position involves leadership in care coordination, promoting optimal clinical outcomes, and effective resource utilization through monitoring hospitalizations and managing utilization review activities.

Minimum Qualifications
  • Bachelor's of Science in Nursing (BSN)
  • Two (2) years recent experience in clinical, utilization management, discharge planning, or case management roles
Skills and Competencies
  • High clinical competence and knowledge of disease processes and treatment protocols
  • Effective communication skills with diverse healthcare stakeholders
  • Ability to organize, prioritize, and problem-solve
  • Demonstrated professionalism and accountability
  • Understanding of patient populations' needs for dignity and respect
Physical Demands
  • Sedentary exertion, up to ten pounds of force occasionally
Compensation and Benefits

Hourly rate: $33.66 - $50.49, based on experience and location. Benefits include health, dental, vision, life insurance, retirement plans, paid time off, and more.
