RN Case Manager Job Opportunity

Seeking a dedicated and experienced Registered Nurse (RN) to fill a part-time Case Manager position. This role involves facilitating patient care plans, collaborating with healthcare teams, and promoting efficient use of healthcare resources.

Responsibilities
  • Leverage leadership skills to drive optimal clinical outcomes, ensure continuity of care, and coordinate patient transfers between different levels of care.
  • Maintain effective utilization of healthcare services by monitoring patient hospitalizations and identifying opportunities for quality improvement.
  • Play a key role in the interdisciplinary team to achieve actions and processes designed to ensure optimal clinical outcomes and cost-effective care.
Qualifications
  • Bachelor's degree in Nursing (BSN)
  • At least two years of experience in clinical, utilization management, home care, discharge planning, managed care, and/or case management that has provided a broad understanding of Case Management and Utilization Management concepts.
