RN Case Manager - Hospice Opportunity

We are seeking a dedicated RN Case Manager to provide high-quality care to patients in their homes and healthcare facilities.

Job Summary:

This role is responsible for planning, coordinating, and delivering quality hospice care while ensuring compliance with federal, state, and local regulations.

Key Responsibilities:
  • Provide ongoing assessment and nursing care to patients, ensuring an accurate evaluation of patient needs and developing comprehensive care plans that address current needs.
  • Follow current nursing professional standards of practice and agency-approved protocols for hospice care.
  • Communicate effectively with healthcare providers about changes in patient condition and needs.
Requirements:
  • A current unencumbered license to practice as a Registered Nurse (RN) in the state of Ohio is required.
  • At least one year of experience in acute care or medical-surgical nursing is required; experience in hospice is preferred.
  • Proficiency with Windows, Microsoft Office, and electronic medical records (EMR) is required.
  • The ideal candidate must be able to read, write, speak, and understand the English language.
