Job Description
We are looking for an energetic, personable, and community-oriented Community Liaison to help grow our local presence and connect individuals and families with the care they deserve. As the face of FirstLight Home Care in the field, you will build trusted relationships with referral partners, generate new client opportunities, and serve as a key contributor to our mission of compassionate, personalized home care.
Key Responsibilities- Develop and nurture relationships with referral sources such as hospitals, physician offices, skilled nursing facilities, senior living communities, social workers, and discharge planners
- Promote FirstLight's services through consistent in-person outreach, presentations, and local community engagement
- Identify, follow up on, and convert leads into active home care clients
- Partner with the care coordination team to ensure a smooth and responsive onboarding experience for clients and their families
- Meet and exceed monthly sales and growth targets aligned with agency goals
- Maintain accurate records in CRM and track referral outcomes and follow-up activity
- Represent FirstLight Home Care at local events, health fairs, and professional networking groups to build brand awareness
- Stay informed about industry trends, local market shifts, and competitor offerings
- Assist with office or administrative tasks as needed, including client communications, marketing support, and occasional in-office duties
- Contribute positively to a collaborative, mission-driven team environment
- 2+ years of outside sales or business development experience in home care, hospice, healthcare, or a related field
- Demonstrated success meeting or exceeding sales targets
- Excellent communication, relationship-building, and public speaking skills
- Self-motivated, highly organized, and comfortable working independently
- Familiarity with local healthcare networks and referral sources is a strong plus
- Reliable transportation and a valid driver's license required
- A sincere passion for senior care and improving the lives of others
- In the field Northeast Cleveland (eastside of Cuyahoga County, Geauga and Lake Counties) with some in-office responsibilities
