  6. Ensure the wider DSA Program meets its administrative, financial, risk, quality, and reporting goals across KPIs.
  7. Provide direct consultation and support to clients as needed.
  8. Ensure timely referral allocation to appropriate staff in line with KPIs.
  9. Report regularly to the Regional Manager on staff performance, service quality, and operational issues.
  10. Recruit suitable Dementia Consultants, Support Coaches, and Family Liaison Officers.
  11. Manage stakeholder relationships with residential aged care, home care, and hospital settings to promote program understanding and effectiveness.
  12. Handle customer complaints or challenging situations promptly, notifying managers when necessary.
  13. Address incidents and feedback related to service quality with the Regional Manager.
  14. Enhance personal knowledge and skills through ongoing learning.
  15. Ensure the safety and maintenance of the DC office environment and fleet vehicles in partnership with other Team Leaders.
  16. Manage financial approvals for brokerage and travel according to policy.

What do we need from you?

  • Commitment to HammondCare's Christian Mission and Values.
  • Qualifications in Nursing, Allied Health, or a related health degree.
  • Experience supporting older adults and carers with dementia, emphasizing psycho-social case management approaches.
